Most people assume the pain is in the joint because the joint is the problem. The knee is a simple hinge. It follows the instructions it receives from the hip above and the ankle below.
When those forces are uneven, the knee absorbs the difference. Over time, that uneven loading produces pain, stiffness and the kind of gradual deterioration that gets labelled as wear and tear.
Wear and tear is a description. It isn't a cause.
Hip pain follows the same logic. The hip sits close to the lumbar spine and the sacroiliac joint, and pain in the hip is frequently coming from one of those structures rather than the hip itself. A scan may show changes in the joint. It won't always tell you why they are there.

Why our approach is different
By the time most people reach us, they've tried a few things. Painkillers, the GP, maybe physio or an injection. Some rest. A scan that showed wear and tear and not much else. Things improve for a while, and then the pain comes back. There's a reason for that. Everything tried so far was aimed at the joint. None of it addressed what's loading the joint unevenly in the first place.
Physio builds strength around the knee. But if the hip mechanics driving the problem haven't changed, the loading pattern hasn't changed either. The knee keeps taking more than its share, and the pain returns. Injections reduce inflammation in the joint. The inflammation comes back because the cause is still there.
The scan is worth addressing too. Imaging shows changes in a joint. It doesn't explain why those changes are there, or why you have pain on one side and not the other, or why it started when it did. Many people have significant wear and tear on imaging and no pain at all. The scan describes the joint. It doesn't diagnose the cause. What we're looking for:
- Which joints in your spine and pelvis aren't moving as they should
- Whether the hip is generating force properly, or passing the load elsewhere
- How your posture and movement patterns are distributing weight through your lower body
- Whether the problem in the joint is the cause, or the consequence
